At Aspirations we are dedicated to supporting individuals with learning disabilities, Autism and / or mental health support needs to move into their own homes, no matter how complex their support requirements may be.

Supporting moves from hospital to home

We support individuals who have previously lived in long-stay hospitals, including those whose histories may involve behaviours such as property damage (a group often overlooked for independent living opportunities).

We can support people who may be managed through complex legal frameworks to progressively move into their own homes as tenants, from hospital.

Removing barriers to independence

We are committed to removing barriers to independence, which is why we collaborate closely with a wider team to get each individual’s unique support right for them. We collaborate closely with Commissioners, Housing Providers, Property Developers, Contractors, and Occupational Therapists to adapt housing design, and create robust environments that meet complex needs. By inputting directly into housing development and adaptation, we ensure that our services are not only fit for purpose but also aligned with the national guidance ‘Building the Right Support’.

Central to our ethos is developing self-resilience using a strengths-based approach. Our focus is to help each individual move forward and develop independence at their pace. We look at what is important to the individual to have a meaningful day and develop new skills that: encourage and promote independence; maintain their home; engage in their community. We firmly believe everyone has huge potential to achieve their goals regardless of any complexities.

Own home, not hospitals

In line with our ‘own home not hospitals’ ethos, Aspirations support starts with a comprehensive assessment of needs. Using this information, we then create and develop a Person-Centred Transition Plan for the individual. All this is done in co-production with the individual and their family / network of support.

Positive Behaviour Support

We are committed to positive behaviour support across all aspects of our care and support, as a golden thread through all we do. We ensure each person’s support is both effective and respectful, our teams work alongside Positive Behaviour Support (PBS) practitioners, delivering care and support that is person-centred and based on the least restrictive practices possible.

Goals we help people achieve

We put the person we are supporting at the centre of the wider collaborative team and support mechanisms, ensuring individual, person specific, person-centred, focussed solutions for each individual.

Multi-disciplinary team

Aspirations embraces multi-disciplinary working with all professionals, such as the receiving community team, discharging placement team, social workers and public agencies.

Sourcing accommodation

We source affordable, suitable and fit for purpose accommodation in preferred locations, to meet the individual’s preferences and needs with trusted developer partners.

Working with Housing Providers

We work  with housing partners to support people to manage their responsibilities under a tenancy.

Support with managing finances

We support people with the management of finances and benefits, as these often change significantly when moving into your own home from hospital.

Practical support of Legal Frameworks

We have knowledge and experience of working with the relevant legal frameworks to enable the individual to transfer from hospital to living in their own home, in the community, and utilising the Mental Health Act and/or Mental Capacity Act, where appropriate.

Recruiting the right team

We recruit and train bespoke support teams with the right skills, values, knowledge, and experience.

Person-Centered Support Plan

We develop a comprehensive Person-Centred Support Plan for each individual, developing self-resilience using a strengths-based, positive risk approach.
